The fees listed below are the amounts charged for services. As a participating provider with Anthem, Aetna, Cigna, Harvard Pilgrim, Community Health Options, Medicare and Medicaid, their allowed fees for some services are lower than the current charges.
EXAMINATIONS
Examination New Patient - $90.00, $130 or $175.00*
Examination New Condition - $60.00, $90.00 or $135*
Re-examination progress assessment - $60 or $90*

Based on the complexity of the condition.
(Medicare does not pay for examination fees - these are out of pocket expenses.)
TREATMENTS
Spinal adjustment of 1 or 2 regions** - $45.00
Spinal adjustment of 3 or 4 regions - $52.00
Spinal adjustment of 5 regions - $67.00

Regions are: cervical (neck), thoracic (mid back), lumbar (lower back), pelvis (hip crests) and sacrum (lowest part of the back)
Non-spinal adjustment/s (jaw, shoulder, elbow, wrist, hip, knee, ankle and/or foot) - $40.00 - includes one or more areas being adjusted.
(Medicare does not pay for non-spinal adjustments so these are an out-of-pocket expense for the patient if treatment is requested/or agreed to.)
Myofascial Release - $40.00
Electrical Stimulation - $40.00
Ultrasound - $40.00
Cold Laser - $30.00. (Insurance does not pay for this service - it is an out-of-pocket expense.)
Mechanical Traction - $40.00
(At this time, Medicare does not cover these services. If the patient agrees to receive any of these services, it is an out-of-pocket expense for the patient.)
Custom Footleveler Orthotics (one pair) - $318.00. (Insurance does not pay for orthotics.)
Footlevelers also offers shoes, sandals, slippers and flip-flops with built-in orthotics - prices vary.
APPOINTMENT CANCELLATION
There is no charge for cancelling an appointment but please give as much notice as possible so that someone else can use that time slot.
NO-SHOW CHARGE
There is a $40 charge for a No-Show for an appointment. We reserve the right to void that charge when circumstances warrant that consideration.
A no-show for a new patient examination will require a prepayment to schedule another new patient examination.
